Background:The decision whether to initiate cardiopulmonary resuscitation may sometimes be ethically complex. While studies have addressed some of these issues, along with the role of nurses in cardiopulmonary resuscitation, most have not considered the importance of nurses acting as advocates for their patients with respect to cardiopulmonary resuscitation.Research objective:To explore what the nurse’s advocacy role is in cardiopulmonary resuscitation from the perspective of patients, relatives, and health professionals in the Basque Country (Spain).Research design:An exploratory critical qualitative study was conducted from (...) October 2015 to March 2016. Thematic analysis was used to analyse the data.Participants:Four discussion groups were held: one with patients and relatives (n = 8), two with nurses (n = 7 and n = 6, respectively), and one with physicians (n = 5).Ethical considerations:Approval was obtained from the Basque Country Clinical Research Ethics Committee.Findings:Three significant themes were identified: (a) accompanying patients during end of life in a context of medical dominance, (b) maintaining the pact of silence, and (c) yielding to legal uncertainty and concerns.Discussion:The values and beliefs of the actors involved, as well as pre-established social and institutional rules reduced nurses’ advocacy to that of intermediaries between the physician and the family within the hospital environment. On the contrary, in primary health care, nurses participated more actively within the interdisciplinary team.Conclusion:This study provides key information for the improvement and empowerment for ethical nursing practice in a cardiac arrest, and provides the perspective of patients and relatives, nurses and physicians. Therefore, we compared two Brazilian editions of Bakhtinian works, with the research on the French author and popular comic culture, with a set of texts involving the Russian original, frame-texts, translations, and other correlated utterances. The reflection is based on a dialogical perspective, particularly on the notion (...) of alterity. The results show that, although corpus and object are a priori amalgamated, it is the essential relationship of the author-researcher with this corpus that gradually detached the object. The article deals with the problem of method for a bioethical approach to clinical cases from the standpoint of bioethics, and analyzes the various methodologies that exist, highlighting their contributions and strengths, as well as their difficulties. Tools for reflecting on how to implement a methodology adapted to the context of contemporary Mexican reality are provided as well. They underscore the characteristics particular to Mexico in bioethical analysis and discernment.

When events such as a pandemic occur, and the entire population’s health must be ensured, the focus is on public health. Despite several criteria for decision-making, it is advisable to delve into the weighting beyond the mere consideration of plainly visible factors. We propose weighting as a mechanism instead of chance since, given the person’s substantial unity and intrinsic dignity, their destiny cannot be subject to anything less than the exercise of intelligible judgment inherent to human reason.

El artículo aborda la relación entre la bioética y la ideología de género a partir de la noción de vulnerabilidad. Así, se afirma que la segunda pone a la persona en una situación de mayor vulnerabilidad al carecer de un fundamento ontológico que la sostenga y, por ende, pudieran desprenderse, de esto, prácticas poco éticas para con quienes se engloban dentro de alguno de los grupos residuales de la misma. Se parte, por tanto, del origen de la ideología de género (...) para, posteriormente, ubicar las problemáticas más frecuentes que se presentan, en el terreno de la Bioética, a partir de la vulnerabilidad antropológica y social. The symbols of the Feminine throughout the dystopian novel The Handmaid’s Tale are the object of study of the article. It is argued that feminine symbology is a unifying thread that makes possible an analysis of the main features of dystopian societies and the possibility of a critical dystopia. The category of analysis is the symbol understood from the perspective of Ricoeur’s hermeneutics of symbols, which proposes that the understanding of a text should be based on what they pretend to (...) say. Therefore, the symbols of the Feminine make possible the resistance within dystopian regimes, and without resistance there is no dystopia. As a result, it is possible through the symbols of the Feminine to interpret the main features of dystopias and, particularly, to develop the possibility of a feminist critical dystopia. (shrink)

This article carried out an analysis from the hermeneutical category of the symbol in the novel Blindness by José Saramago, following Paul Ricoeur, in which the symbols of sin, stain and guilt were chosen to be developed after defining the relevance and the impact of the category of analysis on current situations in our society. Making use not only of the texts referring to Ricoeur's hermeneutics, but also of The Symbolic of Evil, a conceptual journey is proposed to elucidate how (...) the senses and meanings of the symbols of evil are configured in our culture and, through this understanding, address the situation of the symbolic apparatus of Saramago's novel. Carrying out an analysis of these hermeneutical categories in a novel such as Blindness becomes the opportunity to show a different meaning and sense of a novel that has clearly shown its relevance in previous times and that today, in the middle of a pandemic, invites us to rethink that world and to face the fact of understanding our own blindness and the blindness of the world in the midst of a dizzying speed that requires us to assume our reality in another way. In the words of Ricoeur, the symbol gives food for thought and it is time for us to think through our own symbols about the reality that is presented to us in Saramago's novel and to extrapolate it to the realities that are configured every day in our Western world. This article describes and analyzes the design and functioning of the Statistical Ethics System (SETE) in Colombia’s National Statistical Office. It presents the methodology and general process of planning and implementation of the System, supported by a conceptual analysis of the requirements for an ethical functioning of official statistics. The general objective of the article is to make a practical contribution to the understanding of conceptual and practical features that ought to be considered in the implementation of an ethical system (...) in the operation of the public sector, particularly in the area of the production of official statistics. The text is divided into five parts. In the first part, we make a general introduction to the ethical value of official statistics. In the second and third parts, we make a presentation and analysis of the principles and risks that were selected as the basic ethical framework for the production of official statistics at the National Administrative Department of Statistics (DANE). The fourth section presents the conceptual and practical bases of how SETE operates. The fifth section we present some concluding remarks. (shrink)

The acquisition and evolution of speech production, discourse and communication can be negatively impacted by brain malformations. We describe, for the first time, a case of developmental dynamic dysphasia (DDD) in a right-handed adolescent boy (subject D) with cortical malformations involving language-eloquent regions (inferior frontal gyrus) in both the left and the right hemispheres. Language evaluation revealed a markedly reduced verbal output affecting phonemic and semantic fluency, phrase and sentence generation and verbal communication in everyday life. Auditory comprehension, repetition, naming, (...) reading and spelling were relatively preserved, but executive function was impaired. Multimodal neuroimaging showed a malformed cerebral cortex with atypical configuration and placement of white matter tracts bilaterally and abnormal callosal fibers. Dichotic listening showed right hemisphere dominance for language and functional magnetic resonance imaging (fMRI) additionally revealed dissociated hemispheric language representation with right frontal activation for phonology and bilateral dominance for semantic processing. Moreover, subject D also had congenital mirror movements (CMM), defined as involuntary movements of one side of the body that mirror intentional movements of the other side. Transcranial magnetic stimulation and fMRI during voluntary unimanual (left and right) hand movements showed bilateral motor cortex recruitment and tractography revealed a lack of decussation of bilateral corticospinal tracts. Genetic testing aimed to detect mutations that disrupt the development of commissural tracts correlating with CMM (e.g., Germline DCC mutations) was negative. Overall, our findings suggest that DDD in subject D resulted from the underdevelopment of the left inferior frontal gyrus with limited capacity for plastic reorganization by its homologous counterpart in the right hemisphere. Corpus callosum anomalies probably contributed to hinder interhemispheric connectivity necessary to compensate language and communication deficits after left frontal involvement. (shrink)

This article argues that the social constructivist paradigm falls into the same dualistic trap as biological essentialism when attempting to respond to questions of gender and sexuality. I argue that social constructivism, like biological determinism, presumes a ‘split’ world, where subjective lived experiences are separated from the world of socio-cultural forces. Following a phenomenological approach, grounded in Merleau-Ponty’s ontological view of the body, this article attempts to move beyond the dualistic metadiscourses of social constructivism in maintaining that identity is a (...) fully embodied process. I see gender and sexuality as necessarily embodied and corporeally constituted. In the light of this, I propose an understanding of gender and sexuality that focuses on the centrality of the body as open project. This approach sees gender and sexuality as embodied processes that are enmeshed with the complex fabric of lived everyday experiences and concurrent socio-cultural and historical processes. Drawing on real-life examples, I conclude that gender and sexual embodiment are not one-dimensional according to a binary system of male versus female. Rather, given the documented experience of the indeterminacy and ambiguity of human existence, there are a variety of possible embodiments of humankind. (shrink)

O nosso intuito é problematizar o ensino da filosofia como uma disciplina a ser aprendida. Pretendemos desta forma, trazer questões que possam auxiliar a nossa investigação para uma solução coerente e satisfatória. Para isso, retomaremos alguns conceitos gregos de educação, focando-nos no séc. V a.C. através da tragédia e da sofística para demonstrar a influência destas dentro daquilo que chamamos educação. Se tomarmos o mestre como o artífice e o discípulo como objeto deste ensino, devemos ter em mente que a (...) arte de ensinar é conveniente ao discípulo e não ao mestre. O mestre, portanto, é instrumento para que se possa ensinar tal arte ao discípulo. O objetivo deste trabalho é refletir o ensino, seu instrumento e seu objeto com o intuito de repensar o caminho da filosofia na educação. Apresenta investigação sobre a relação entre a Representação da Informação e a Dialética, demonstrando que enquanto campos de estudos podem contribuir para um repensar da forma como o conhecimento pode se estabelecer. Traz como questão-chave: Por qual motivo devemos fazer uso dos princípios que regem a Dialética para representar cibercordéis? Para responder a esse problema de pesquisa, tem-se como objetivo geral: Apresentar a Dialética como uma proposta teórico-metodológica capaz de auxiliar no processo de Representação da Informação de cibercordéis. Metodologicamente, a (...) investigação é qualitativa, trazendo como a abordagem o próprio Campo da Dialética e corpus, os Cordéis Digitais (cibercordéis) presentes em grupos de facebook com buscas orientadas pela hashtag cordel (#cordel). Nos resultados identificou-se a presença de 8 (oito) Marcadores Sociais, a saber: Classe Social, Étnico-racial, Gênero, Geração, Nacionalidade, Regionalidade, Religião e Sexualidade. Uma vez categorizados, com a análise dos dados, foi possível melhorar o processo de Representação da Informação através do uso do princípio da contradição, por exemplo, e assim, tornar a prática informacional mais robusta e eficaz. Nas conclusões, foi possível chegar a um posicionamento basilar para podermos compreender a ideia do que é Representação Dialética da Informação, especialmente, se tomado como ponto de apoio e partida, princípios como os da contradição e o da totalidade. Our goal in this paper is to articulate a precise concept of at least a certain kind of disease-mongering, showing how pharmaceutical marketing can commercially exploit certain diseases when their best definition is given through the success of a treatment in a clinical trial. We distinguish two types of disease-mongering according to the way they exploit the definition of the trial population for marketing purposes. We argue that behind these two forms of disease-mongering there are two well-known problems in the (...) statistical methodology of clinical trials (the reference class problem and the distinction between statistical and clinical significance). No two individuals with the autism diagnosis are ever the same—yet many practitioners and parents can recognize signs of ASD very rapidly with the naked eye. What, then, is this phenotype of autism that shows itself across such distinct clinical presentations and heterogeneous developments? The “signs” seem notoriously slippery and resistant to the behavioral threshold categories that make up current assessment tools. Part of the problem is that cognitive and behavioral “abilities” typically are theorized as high-level disembodied and modular functions—that (...) are assessed discretely (impaired, normal, enhanced) to define a spectral syndrome. Even as biology reminds us that organic developing bodies are not made up of independent switches, we remain often seduced by the simplicity of mechanistic and cognitive models. Developmental disorders such as autism have accordingly been theorized as due to different modular dysfunctions—typically of cortical origin, i.e., failures of “theory of mind” (Baron-Cohen et al., 1985), of the “mirror neuron system” (Ramachandran and Oberman, 2006), of “weak central coherence” (Happe and Frith, 2006) or of the balance of “empathizing” and “systemizing” (Baron-Cohen, 2009), just to list a few. -/- The broad array of autonomic (Ming et al., 2005; Cheshire, 2012) and sensorimotor (Damasio and Maurer, 1978; Maurer and Damasio, 1982; Donnellan and Leary, 1995; Leary and Hill, 1996; Donnellan and Leary, 2012; Donnellan et al., 2012) differences experienced and reported by people with autism have by such theories typically been sidelined as “co-morbidities,” possibly sharing genetic causes, but rendered as incidental and decisively behaviorally irrelevant symptoms—surely disconnected from cognition. But what if the development of cortically based mental processes and autonomous control relies on the complexities and proper function of the peripheral nervous systems? Through such an “embodied” lens the heterogeneous symptoms of autism invites new interpretations. We propose here that many behavioral-level findings can be re-defined as downstream effects of how developing nervous systems attempt to cope and adapt to the challenges of having various noisy, unpredictable, and unreliable peripheral inputs. Background: Serious games have shown positive results in increasing motivation, adherence to treatment and strengthening the therapeutic alliance in multiple psychiatric disorders. In particular, patients with impulse control disorders and other disorders in which the patient suffers from inhibitory control deficits have been shown to benefit from serious games.Aim: The aim of this study was to describe the characteristics and to evaluate the usability of a new serious videogame, e-Estesia. This serious videogame was designed to improve emotion regulation in patients (...) with gambling disorder. Preliminary results from a pilot sample are also reported.Method: A pilot sample of 26 patients undergoing treatment for GD was recruited. Participants used e-Estesia on a tablet, which was connected to a thoracic band that sent heart rate and heart rate variability data to the videogame platform in order to provide biofeedback. The System Usability Scale was completed by patients to determine the usability of e-Estesia.Results and Discussion: e-Estesia performed comparatively well for all the explored groups. Around 84.6% of the patients endorsed that it was easy to use. Female patients with GD presented higher HRV during the use of the serious videogame compared to men. (shrink)

This article is an extended promenade strolling along the winding roads of identity, equality, nameability and completeness, looking for places where they converge. We have distinguished between identity and equality; the first is a binary relation between objects while the second is a symbolic relation between terms. Owing to the central role the notion of identity plays in logic, you can be interested either in how to define it using other logical concepts or in the opposite scheme. In the first (...) case, one investigates what kind of logic is required. In the second case, one is interested in the definition of the other logical concepts in terms of the identity relation, using also abstraction. The present paper investigates whether identity can be introduced by definition arriving to the conclusion that only in full higher-order logic a reliable definition of identity is possible. However, the definition needs the standard semantics and we know that with this semantics completeness is lost. We have also studied the relationship of equality with comprehension and extensionality and pointed out the relevant role played by these two axioms in Henkin’s completeness method. We finish our paper with a section devoted to general semantics, where the role played by the nameable hierarchy of types is the key in Henkin’s completeness method. (shrink)

Autism has been defined as a disorder of social cognition, interaction and communication where ritualistic, repetitive behaviors are commonly observed. But how should we understand the behavioral and cognitive differences that have been the main focus of so much autism research? Can high-level cognitive processes and behaviors be identified as the core issues people with autism face, or do these characteristics perhaps often rather reflect individual attempts to cope with underlying physiological issues? Much research presented in this volume will point (...) to the latter possibility, i.e. that people on the autism spectrum cope with issues at much lower physiological levels pertaining not only to Central Nervous Systems (CNS) function, but also to peripheral and autonomic systems (PNS, ANS) (Torres, Brincker, et al. 2013). The question that we pursue in this chapter is what might be fruitful ways of gaining objective measures of the large-scale systemic and heterogeneous effects of early atypical neurodevelopment; how to track their evolution over time and how to identify critical changes along the continuum of human development and aging. We suggest that the study of movement variability—very broadly conceived as including all minute fluctuations in bodily rhythms and their rates of change over time (coined micro-movements (Figure 1A-B) (Torres, Brincker, et al. 2013))—offers a uniquely valuable and entirely objectively quantifiable lens to better assess, understand and track not only autism but cognitive development and degeneration in general. This chapter presents the rationale firstly behind this focus on micro-movements and secondly behind the choice of specific kinds of data collection and statistical metrics as tools of analysis (Figure 1C). In brief the proposal is that the micro-movements (defined in Part I – Chapter 1), obtained using various time scales applied to different physiological data-types (Figure 1), contain information about layered influences and temporal adaptations, transformations and integrations across anatomically semi-independent subsystems that crosstalk and interact. Further, the notion of sensorimotor re-afference is used to highlight the fact that these layered micro-motions are sensed and that this sensory feedback plays a crucial role in the generation and control of movements in the first place. In other words, the measurements of various motoric and rhythmic variations provide an access point not only to the “motor systems”, but also access to much broader central and peripheral sensorimotor and regulatory systems. Lastly, we posit that this new lens can also be used to capture influences from systems of multiple entry points or collaborative control and regulation, such as those that emerge during dyadic social interactions. There has been an increase in cognitive assessment via the Internet, especially since the coronavirus disease 2019 surged the need for remote psychological assessment. This is the first study to investigate the appropriability of conducting cognitive assessments online with children with a neurodevelopmental condition and intellectual disability, namely, Williams syndrome. This study compared Raven’s Colored Progressive Matrices and British Picture Vocabulary Scale scores from two different groups of children with WS age 10–11 years who were assessed online or face-to-face. Bayesian (...) t-tests showed that children’s RCPM scores were similar across testing conditions, but suggested BPVS scores were higher for participants assessed online. The differences between task protocols are discussed in line with these findings, as well as the implications for neurodevelopmental research. (shrink)

The current assessment of behaviors in the inventories to diagnose autism spectrum disorders (ASD) focus on observation and discrete categorizations. Behaviors require movements, yet measurements of physical movements are seldom included. Their inclusion however, could provide an objective characterization of behavior to help unveil interactions between the peripheral and the central nervous systems. Such interactions are critical for the development and maintenance of spontaneous autonomy, self-regulation and voluntary control. At present, current approaches cannot deal with the heterogeneous, dynamic and stochastic (...) nature of development. Accordingly, they leave no avenues for real-time or longitudinal assessments of change in a coping system continuously adapting and developing compensatory mechanisms. We offer a new unifying statistical framework to reveal re-afferent kinesthetic features of the individual with ASD. The new methodology is based on the non-stationary stochastic patterns of minute fluctuations (micro-movements) inherent to our natural actions. Such patterns of behavioral variability provide re-entrant sensory feedback contributing to the autonomous regulation and coordination of the motor output. From an early age, this feedback supports centrally driven volitional control and fluid, flexible transitions between intentional and spontaneous behaviors. We show that in ASD there is a disruption in the maturation of this form of proprioception. Despite this disturbance, each individual has unique adaptive compensatory capabilities that we can unveil and exploit to evoke faster and more accurate decisions. Measuring the kinesthetic re-afference in tandem with stimuli variations we can detect changes in their micro-movements indicative of a more predictive and reliable kinesthetic percept. Our methods address the heterogeneity of ASD with a personalized approach grounded in the inherent sensory-motor abilities that the individual has already developed. (shrink)

Sin embargo, como explicamos en este trabajo, los presupuestos conceptuales en que apoya su interpretación del derecho a la vida privada son discutibles (...) y, en consecuencia, es dudoso que este derecho sirva de fundamento, de “caballo de Troya”, para legitimar/legalizar la eutanasia (activa, particularmente). Tomando como referente los análisis de Michel Foucault en torno al biopoder ilustramos cómo de los distintos discursos (sociales, políticos y jurídicos) en torno a la maternidad subrogada se desprenden determinadas pautas dominantes que serían (...) evidencias de un poder que se ejerce sobre la vida humana natural, no mediante la coerción sino mediante su exaltación y su incitación. The current rise of neurodevelopmental disorders poses a critical need to detect risk early in order to rapidly intervene. One of the tools pediatricians use to track development is the standard growth chart. The growth charts are somewhat limited in predicting possible neurodevelopmental issues. They rely on linear models and assumptions of normality for physical growth data – obscuring key statistical information about possible neurodevelopmental risk in growth data that actually has accelerated, non-linear rates-of-change and variability encompassing skewed distributions. Here, (...) we use new analytics to profile growth data from 36 newborn babies that were tracked longitudinally for 5 months. By switching to incremental (velocity-based) growth charts and combining these dynamic changes with underlying fluctuations in motor performance – as the transition from spontaneous random noise to a systematic signal – we demonstrate a method to detect very early stunting in the development of voluntary neuromotor control and to flag risk of neurodevelopmental derail. This article is a continuation of our promenade along the winding roads of identity, equality, nameability and completeness. We continue looking for a place where all these concepts converge. We assume that identity is a binary relation between objects while equality is a symbolic relation between terms. Identity plays a central role in logic and we have looked at it from two different points of view. In one case, identity is a notion which has to be defined and, in the (...) other case, identity is a notion used to define other logical concepts. In our previous paper, [16], we investigated whether identity can be introduced by definition arriving to the conclusion that only in full higher-order logic with standard semantics a reliable definition of identity is possible. In the present study we have moved to modal logic and realized that here we can distinguish in the formal language between two different equality symbols, the first one shall be interpreted as extensional genuine identity and only applies for objects, the second one applies for non rigid terms and has the characteristic of synonymy. We have also analyzed the hybrid modal logic where we can introduce rigid terms by definition and can express that two worlds are identical by using the nominals and the @ operator. We finish our paper in the kingdom of identity where the only primitives are lambda and equality. Here we show how other logical concepts can be defined in terms of the identity relation. We have found at the end of our walk a possible point of convergence in the logic Equational Hybrid Propositional Type Theory, [14] and [15]. (shrink)

This chapter examines Bunge’s distinction between the logical concept of existence and the ontological one. We introduce a new conceptual existence predicate in an intensional environment that depends on the evaluation world. So that we can investigate restricted areas where the different kinds of concepts might exist. We hope this new predicate would encompass Bunge’s philosophical position which he designates as conceptualist and fictional materialism. The basic hybridization acts as a bridge between intensions and extensions because @ works as a (...) useful rigidifier. In hybrid logic, the accessibility relation and many properties this relation might have can be easily expressed in the formal language. The initial hypothesis is that hybridization and intensionality can serve as unifying tools in the areas involved in this research; namely, Logic, Philosophy of Science and Linguistics. (shrink)

Patients’ beliefs about the effectiveness of their treatments are key to the success of any intervention. However, since these beliefs are usually formed by sequentially accumulating evidence in the form of the covariation between the treatment use and the symptoms, it is not always easy to detect when a treatment is actually working. In Experiments 1 and 2, we presented participants with a contingency learning task in which a fictitious treatment was actually effective to reduce the symptoms of fictitious patients. (...) However, the base-rate of the symptoms was manipulated so that, for half of participants, the symptoms were very frequent before the treatment, whereas for the rest of participants, the symptoms were less frequently observed. Although the treatment was equally effective in all cases according to the objective contingency between the treatment and healings, the participants’ beliefs on the effectiveness of the treatment were influenced by the base-rate of the symptoms, so that those who observed frequent symptoms before the treatment tended to produce lower judgments of effectiveness. Experiment 3 showed that participants were probably basing their judgments on an estimate of effectiveness relative to the symptom base-rate, rather than on contingency in absolute terms. This editorial explains why we are launching Research Integrity and Peer Review, a new open-access journal that will provide a home to research on ethics, reporting, and evaluation of research. We discuss how the idea to launch this journal came about and identify the gaps in knowledge where we would like to encourage more research and interdisciplinary discussion. We are particularly keen to receive submissions presenting actual research that will increase our understanding and suggest potential solutions to issues related to (...) peer review, study reporting, and research and publication ethics. This theoretical paper proposes a new perspective to understand the moral distress of nurses more fully, using virtue ethics. Moral distress is a widely studied subject, especially with respect to the determination of its causes and manifestations. Increasing the theoretical depth of previous work using ethical theory, however, can create new possibilities for moral distress to be explored and analyzed. Drawing on more recent work in this field, we explicate the conceptual framework of the process of moral distress in nurses, (...) proposed by Ramos et al., using MacIntyrean virtue ethics. Our analysis considers the experience of moral distress in the context of a practice, enabling the adaptation of this framework using virtue ethics. The adoption of virtue ethics as an ethical perspective broadens the understanding of the complexity of nurses’ experiences of moral distress, since it is impossible to create a ready model that can cover all possibilities. Specifically, we describe how identity, social context, beliefs, and tradition shape moral discomfort, uncertainty, and sensitivity and how virtues inform moral judgments. Individuals, such as nurses, who are involved in a practice have a narrative history and a purpose ( telos) that guide them in every step of the process, especially in moral judgment. It is worth emphasizing that the process described is supported by the formation of moral competence that, if blocked, can lead to moral distress and deprofessionalization. It is expected that nurses seek to achieve the internal good of their practice, which legitimizes their professional practice and supports them in moral decision-making, preventing moral distress.
